Avon Old Farms Baseball Camp

We hope that you are looking forward to the start of summer and attending AOF Baseball Camp. As preparations continue, we wanted to send out a reminder of items that need to be taken care of prior to the start of camp along with general camp information.

Prior to Camp:

• Final payment is due by June 30, 2014. Payments can be sent to the address listed below. Please remember to list your child’s name on the memo line of the check.

• Medical waiver and emergency forms must be received by June 30th. There are three mandatory forms required: 1) Medical Liability Waiver and Release Form, 2) Health History Form and 3) Emergency Contact. You may submit an alternative health history form so long as it is attached to our form and it shows your child’s health history, immunization record, and dated within the past two years. These forms can be found on the Summer Program page of our school website by clicking here.  If your child takes any medication (prescription or non-prescription including, but not limited to inhalers, epi-pens or oral medication), the Self –Administration and Prescription/Non-Prescription forms can be found on this link as well. All medication will be collected at time of registration and held by the camp first aid personnel. Medication needs to be in its original container and clearly labeled with campers name and dosage amount. Your child must be able to self-administer any medication that he or she may take during the camp

What to Bring:

• Cleats and/or sneakers
• Glove and bat
• Labeled water bottle
• Lunch – bring a small labeled cooler

Campers will be dropped-off and picked-up at the Avon Old Farms baseball field located off Old Farms Road next to Fisher Meadows. Extended hours (8:15 AM to 3:45 PM) are offered but must be arranged prior to the start of camp.
